Summary


Come and join our amazing Children’s Community Nursing Service as a Children’s Community Asthma Nurse specialist. If successful, you will be instrumental in supporting development and delivery of a shared care pathway between the community and acute setting. One which collaborates with the wider North West London Asthma network to enable tangible change for Children and Young people and their health outcomes. Our Asthma team deliver primarily clinic-based care, carrying out respiratory assessment, inhaler use and technique, diagnostic testing and education. A new clinic which is supported by both community and acute settings is moving towards a more targeted approach to effectively manage children and young people with Asthma.



Our nurses need to work autonomously, managing caseloads to assess, implement and evaluate care. High standards of care and competence underpin our service. This clinical ability and confidence support education of wider children’s services.

· Manage and run community Asthma clinics

· Assess, implement and evaluate care including diagnostic testing

· Co-manage complex asthma patient caseload

· Support education programme delivery

· Demonstrate competence and excellence in clinical care delivery

· Work collaboratively as part of the wider Children’s community

Nursing Service and within children’s services

· Work collaboratively with multidisciplinary teams to deliver Asthma

care to meet the needs of Hounslow's Children and young people.

· Support the North West London Asthma network by delivering core

shared initiatives

· Develop and participate in a shared care pathway between acute and

community services.

· Demonstrate competence using IT based systems

· Demonstrate excellent team work and communication
