Summary
This role is suitable for someone who is passionate about developing and providing high quality, evidence based care and treatment to young people with moderate to severe emotional and mental health problems. The post holder will work closely with the Mindworks Alliance including School Based Needs, Surrey Well Being Partnership and other agencies during office hours, responding to referrals of young people presenting in need: To work independently or jointly with Mindworks Hubs staff to provide specialist assessments and treatment to children, adolescents and their families requiring more help or risk support. The post holder will work as an autonomous practitioner, undertaking clinical assessment of children and young people who may have a complex co-morbid mental health need, working alongside the wider network and providing consultations where necessary To be an integral part of the Hub duty Rota, responding to queries from families and the wider network including risk support To work within a highly experienced MDT involved in the assessment and treatment of CYP with moderate to severe and complex mental health difficulties. To manage own caseload from referral to discharge/transfer, holding medium to high risk cases. . Ensure that outcome measures for the team are implemented and audited. Contribute to relevant research and ensure that interventions on the pathway are evidence based. Be responsible for the oversight and completion of initial assessment and subsequent facilitation of a warm handover and referrals to other services and/or discharge with a signed care plan regarding safety and recorded on the electronic patient record. They will also be expected to provide a leadership role by supporting, supervising and mentoring junior colleagues.
