Summary
To manage a defined caseload of service users with a range of complex mental health problems within the community setting. The post holder will be responsible for the assessment, planning, implementation, and evaluation of care programmes, working autonomously and as a key member of the multidisciplinary team. The role requires a high standard of clinical expertise, risk management, and a commitment to recovery-focused practice. 2. Main Duties and Responsibilities Clinical Practice: To undertake comprehensive assessments of service users, including those with complex mental health needs, to determine their health, social care, and risk profile. To develop, implement, and evaluate evidence-based, person-centred care plans and treatment pathways in collaboration with service users, their families/carers, and the multidisciplinary team. To provide a range of specialist therapeutic interventions appropriate to the service users needs. To regularly review and monitor care plans, making adjustments as necessary to support recovery and well-being. Caseload Management: To efficiently manage and prioritise a personal caseload, ensuring timely and effective coordination of care. To maintain accurate, confidential, and up-to-date clinical records in accordance with professional and service standards. Risk Assessment and Management: To conduct systematic risk assessments, formulating and implementing robust risk management plans to safeguard service users and others. To proactively manage crisis situations, providing support and intervention as required. Multidisciplinary Team Working: To work cohesively as part of a multidisciplinary team (MDT), contributing to team decisions, case discussions, and clinical reviews. To communicate effectively with team members, GPs, and other health and social care agencies to ensure integrated care. Professional Development: To maintain and develop own professional knowledge and skills, in line with revalidation requirements. To contribute to the development of others through supervision, mentorship, and sharing of best practice. 3. Key Working Relationships Service Users, their families, and carers. CMHT Multidisciplinary Team (including Psychiatrists, Nurses, Psychologists, Occupational Therapists, and Support Workers). GPs and other Primary Care services. Community and Voluntary Sector organisations. Inpatient Mental Health Services.
