Hertfordshire Community NHS Trust is seeking aClinical Directorto lead our innovativeHospital@Home service, providing high-quality, hospital-level care to patients in their own homes across East and North Hertfordshire.

About the Role

This is an exciting opportunity to join atriumvirate leadership teamalongside a Nursing Lead and an Operational Lead. Together, you will shape and deliver a responsive, safe, and effective service that supports early discharge, prevents unnecessary hospital admissions, and enhances patient experience.

The role is initially offered as asecondment or fixed-term contract, whilst awaitingRoyal College approval for consultant applicants. Once approval is received, the post will be advertised as asubstantive position. We are happy to have a discussion about substantive employment whilst waiting for approval.

The Clinical Director will work5–10 sessions per week, with flexibility to accommodate clinical commitments and service needs.

Key Responsibilities
• Provide clinical oversight and strategic direction for the Hospital@Home service
• Lead on clinical governance, quality improvement, and patient safety
• Collaborate with system partners to develop integrated care models
• Support workforce development and clinical supervision
• Champion innovation and evidence-based practice

About You

We’re looking for a dynamic and experienced clinician (e.g. Consultant, GP, or senior specialist) with:
• Proven leadership in community or acute care settings
• Strong understanding of virtual wards and hospital-at-home models
• Excellent communication and stakeholder engagement skills
• Commitment to patient-centered care and service transformation

RESPONSIBILITIES / ACCOUNTABILITIES:

· To provide senior professional clinical leadership within the triumvirate, provide leadership to the Hospital @ Home Service and contribute to developing the strategic direction of the service.

· To provide strategic advice and leadership and contribute on medical and clinical matters to the Hospital @ Home working groups.

· To be accountable for clinical governance within the service in partnership with the Lead Nurse.

· To support the recruitment, retention and professional development of clinical staff including induction, job planning, appraisals, clinical supervision and organising teaching & training sessions.

· To drive transformation in care delivery, including proactive care models and integration across the community and urgent emergency services.

· To contribute to the development of innovative and effective ways of working to create strong partnerships and relationships with key stakeholders within the system.

· To support team members in delivering on their functionally relevant objectives through offering advice, guidance and support as appropriate.

· To build and sustain effective communications with all members of the multidisciplinary team, and the wider clinical teams across the community service.

· To represent Hospital @ Home at various fora developing partnerships, sharing bestpractice and integrating this knowledge in service improvement.

· To ensure there is clinical engagement in the day-to-day activities of Hospital @ Home, in performance improvements and service developments, working with the leads in other services across the Trust.
