Summary

We are looking for an Adult's Social Worker to join a Hospital Team.

This role requires a Social Work Qualification with a minimum of 2 years post qualified experience.

About the team

This is a fast paced team who work to ensure patients are safely discharged from hospital and have enough support as they need. Carrying out bedside assessments, arranging appropriate care packages for comprehensive discharges and advocating for service users and their independence is at the forefront of the team's motivation. Being able to ensure service users have access to necessary resources and external support at the earliest stages is key to the success of this role.

About you

The successful candidate will have worked in a hospital team or short term assessment setting. A valid UK driving licence and car is not essential for this role. A degree within Social Work (Degree/DipSW/CQSW) with a minimum of two years' experience working in an Adult's Social Work environment is essential as the role is based in two hospitals both 10 minutes from the civic centre.
