Summary
We are looking for a skilled and motivated Practice Nurse to join our friendly, supportive team. This is a varied role with the opportunity to make a real difference to our patients. Key Responsibilities: Provide high-quality care for patients with acute and chronic conditions, including chronic disease management, vaccinations, wound care, cervical smears, and minor procedures. Review and monitor patients with long-term conditions, supporting them to follow treatment plans effectively. Deliver health promotion and wellbeing initiatives to improve patient outcomes. Support frail and housebound patients through effective care management programmes. Assist GPs with minor surgery and other clinical procedures as required. Contribute to infection prevention, safeguarding, QOF and practice safety protocols. Maintain accurate patient records and use Read/SNOMED coding effectively. Support and mentor the nursing team, contributing to training, shared learning, and service improvements. Skills & Experience: Registered Nurse with experience in general practice or community nursing. Confident working independently and collaboratively. Committed to delivering patient-centred care.
