We’re seeking a dedicated and organised Care Coordinator to join our growing team and help us deliver outstanding, person-centred care within the community.
About the Role:
As a Care Coordinator, you will play a key role in ensuring the smooth day-to-day running of our care service. You’ll be responsible for:
Coordinating care visits and managing staff rotas efficiently
Liaising with care staff, service users, families, and external professionals
Ensuring continuity and high-quality care for every service user
Responding to last-minute changes and maintaining service delivery standards
Supporting recruitment, inductions, and ongoing training of care staff
Participating in on-call duties on a rota basis
About You:
We’re looking for someone who is proactive, professional, and passionate about making a difference. You should have:
✅ Minimum 1 year of experience in a care coordination or similar role within domiciliary care
✅ Excellent communication and organisational skills
✅ A good understanding of CQC regulations and care sector standards
✅ The ability to work under pressure and handle scheduling challenges confidently
✅ Competence using electronic care management systems (training can be provided)
✅ A flexible approach to working, including participation in on-call cover
