Summary
Clinical Responsibilities: Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation Consult with patients in the practice, at home visits, via telephone or on-line mechanisms in line with Practice policy Assessing the health care needs of patients with undifferentiated and undiagnosed problems, assessing their health care needs Complete clinically related administrative and non-clinical duties needed for the delivery of the service. Screening patients for disease risk factors and early signs of illness In consultation with patients and in line with current practice and disease management protocols, develop care plans for managing patients' health care needs Providing counselling and health education as necessary Recording clear and contemporaneous consultation notes to agreed standards Contribute to the supervision of both Doctors in Training and medical students Compiling and issuing computer-generated acute and repeat prescriptions, prescribing in accordance with the practice prescribing policy Comply with all relevant clinical governance and Practice polices and protocols Meet professional learning and CPD requirements. Develop effective relationships with other colleagues. Demonstrate commitment to lifelong learning, and audit and effectiveness to ensure evidence based best practice.
