Summary
JOB TITLE: Advanced Nurse Practitioner HOURS: 27 hours per week (Days TBC) REPORTS TO: Clinical: The Partners Administrative: HR Lead Interview Date:17/11/2025 Job responsibilities: Clinical Nursing Practice The Advanced Nurse Practitioner (ANP) will deliver high-quality, patient-centred care within the practice by: Working collaboratively within a multi-disciplinary team to support the health and well-being of a diverse patient population. Serving as the first point of contact for patients with undifferentiated conditions; conducting thorough assessments, diagnosing, and developing management plans. Managing acute and chronic conditions using evidence-based drug and non-drug treatments tailored to individual needs. Providing holistic care focused on prevention, self-care, and health promotion for patients and families. Prescribing and reviewing medications within scope, advising on side effects and interactions. Coordinating patient care, involving patients and carers in decisions, and supporting adherence to treatments. Requesting and interpreting diagnostic tests under agreed protocols to inform clinical decisions. Prioritising and triaging patient needs, managing urgent and emergency situations effectively. Offering mental health assessment and referrals per national guidelines. Delivering care both in practice and community settings, making referrals when needed. Maintaining accurate, contemporaneous clinical records consistent with NMC and practice standards. Supporting practice contractual requirements and contributing to performance monitoring such as QOF and KPIs. Leading and mentoring nursing staff, promoting evidence-based practice. Care Management Communicating sensitive information effectively to patients, relatives, and carers. Providing specialised advice and making referrals, including fast-track where necessary. Contributing to local and national clinical targets and monitoring care outcomes. Health Improvement Managing health screening programmes and promoting health priorities within the team. Introducing innovations to improve patient outcomes and supporting disease prevention efforts. Making appropriate referrals to statutory and voluntary agencies. Administration & Professional Responsibilities Producing accurate, complete patient records and ensuring clinical systems are up-to-date. Ensuring correct completion of documentation and claims related to patient care. Participating in practice meetings and maintaining clinical environments. Training & Personal Development Maintaining advanced clinical skills and professional knowledge through continuous learning. Engaging in appraisals, peer review, clinical supervision, and self-reflection to improve care delivery. Contributing to staff education and student training programs. Developing and maintaining a Personal Learning Plan in line with revalidation requirements. Confidentiality Complying with data protection laws and maintaining strict confidentiality of patient, staff, and practice information. Health & Safety Identifying, reporting, and managing health and safety and infection control risks. Following national standards for infection control, sharps management, waste disposal, and spillage control. Promoting safeguarding and welfare of children and vulnerable adults.
