Summary
Key responsibilities 1.Strategic Leadership 1.1.Lead the development and implementation of the clinical transformation strategy, aligned with the Hospices vision and objectives. 1.2.Work with internal and external colleagues to identify opportunities for innovation and improvement in clinical services, ensuring best practice and evidence-based care. 1.3.Champion the Hospices 2030 ambition to reach more people, reduce unmet need, and become a sustainable, integrated part of the health and social care system 1.4.Work with senior leaders to set transformation priorities and measure impact. 1.5.To be a core member of the leadership team within Patient Care and Communities, and the wider Extended Leadership Group 2.Operational Delivery 2.1.Deliver transformation projects, ensuring milestones, outcomes, and benefits are achieved on time and within budget. 2.2.Collaborate with clinical and non-clinical teams to embed new models of care, pathways, and processes. 2.3.Monitor and report on project progress, risks, and outcomes to the executive team and Board as required. 2.4.Promote outcome-focused care, moving beyond activity metrics to measure impact and effectiveness. 2.5.Embed co-production and inclusive design principles into service transformation 3.Change Management 3.1.Engage and support staff, volunteers, patients, and families through change, ensuring clear communication and involvement. 3.2.Provide training, coaching, and support to teams to build transformation capability across the organisation. 3.3.Build system capacity and act as an influential leader within the organisation and wider system 3.4.Strengthen relationships with statutory and voluntary sector partners, universities, and community groups. 3.5.Share specialist knowledge and insights to support wider system improvement 4.Quality and Governance 4.1.Ensure all transformation initiatives comply with regulatory and quality standards 4.2.Use data and feedback to evaluate the effectiveness of changes and drive continuous improvement. 4.3.To ensure that benefits are reported and tracked on monthly basis as the projects progress. 4.4.To provide early warning on risks and issues relating to programmes as they emerge. 4.5.Ensure projects and initiatives are delivered on time, to quality standards and in a cost effective manner, adjusting plans as required in collaboration with the Director of Patient Care and Communities 4.6.Monitor and evaluate transformation initiatives, reporting on outcomes and impact.
