We are seeking a dynamic and motivated Band 7 Physiotherapist (or an experienced Band 6 looking to progress) with strong assessment and treatment skills in stroke and neurological rehabilitation to join our Stroke Therapies Service.

This is an exciting opportunity to work within a supportive multidisciplinary team, delivering high-quality, patient-centred care. You will play a key role in the rehabilitation journey of patients with complex neurological needs, contributing to innovative service development and clinical excellence.

What we’re looking for:
• Proven experience in stroke and/or neurological rehabilitation
• Demonstrates a willingness to participate in the emergency on-call and weekend respiratory physiotherapy service, working autonomously when undertaking these duties
• Excellent clinical reasoning and decision-making skills
• A collaborative approach to multidisciplinary working
• Commitment to continuous professional development and service improvement

If you’re ready to make a meaningful impact and grow within a forward-thinking team, we’d love to hear from you.
• To provide a high standard physiotherapy service to stroke and neurologically impaired adult inpatients.
• To undertake advanced assessment and treatment of stroke and neurologically impaired and other in-patients including those who have highly complex and / or chronic presentations, to determine clinical diagnosis and treatment indicated and to maintain associated records, as an autonomous practitioner
• To provide highly specialised advice to patients, family, carers, and health and social care professionals regarding the management of patients with highly complex neurological conditions.
• Interpret and analyse clinical and non-clinical facts, to provide accurate recommendations on further needs and development of comprehensive discharge plans.
• To take a lead role in the case management of patients with highly complex neurological disabilities and who require onward referral to specialist Neuro-rehabilitation and where appropriate, facilitation of transfer to appropriate interim facilities.
• Communicate with the MDT and the patient / family re: the status of rehabilitation referrals and requirements for acceptance / transfer. This includes highlighting requirements to support referrals (completion of reports, nutritional management e.g. insertion of PEG, setting and implementation of MDT goals) to the MDT.

Surrey Downs Health and Care deliver care closer to people’s own communities through our Primary Care Networks, Community Hospitals, Specialist Services and our innovative partnership of local NHS organisations.

Surrey Downs Health and Care has a track record of providing person centered care that goes beyond organisational boundaries to do what is best for the individual. This partnership includes:
• The three GP federations GP Health Partners, Dorking Health Care and Surrey Medical Network representing practices that operate in the Surrey Downs area
• Epsom and St Helier’s University Hospitals NHS Trust
• Surrey Council County

Historically, there have been boundary lines between the organisations that provide care to people in their homes, in GP surgeries and in hospitals, but we have always been united in our mission to provide great care to the people who need us.

It’s on those grounds that the Surrey Downs Health and Care was formed – we want local people to receive the care that they need in the right environment. By bringing together our expertise, we can improve patient care and enable local people to access the right support, care and treatment more easily than ever before.

In bringing this partnership together, we are working to the same set of values that will translate into better care for our residents.
