Summary

Key Responsibilities Support the Infection Control Lead and work to the IPC policy. Undertake Holistic patient assessments with patient and their families. Support long term condition management including involvement with quality improvement and assurance initiatives. Teaching patients and families how to manage their condition. Support the management of care for frail and housebound patients. Support public health screening programmes including immunisation, vaccination, cervical screening and provide opportunistic health promotion advice. Support population health management targeting health inequalities and improving access. Provide routine nursing care in accordance with clinical based evidence, NICE and NSF. Participate in clinical audit and research projects including supporting development and updating of practice protocols and guidelines. Work within national and local protocols. Recognise boundaries of practice and know when and to whom patients should be referred. Support delivery of enhanced services and other service requirements. Be aware of responsibilities under current safeguarding of adults and children policies and adhere to policies and procedures. Participate in learning from complaints, incidents and near miss events. Undertake all mandatory training and contribute to clinical governance. Provide basic wound care in line with local enhanced services. Maintain accurate clinical records using data entry templates where available ensuring SNOMED codes are used effectively. Contribute to practice targets including QOF and Local Enhanced services. Maintain and clean, tidy and effective work area at all times. Chaperone patients when requested by GP's, Locums or other nursing staff. Support the team with clinical emergencies. Liaise with external services to ensure that patients are supported appropriately (vulnerable patients, etc). You may be on occasion required to carry out other tasks dependent on factors including workload and staffing levels. You may on occasion be asked to extend your working day to cover for an absent colleague at short notice.
