Summary

Are you passionate about supporting people with a learning disability to live fulfilling lives in their communities? Do you thrive on building partnerships and leading change? If so, we’d love you to join our NI Development Team as a Supported Living Transition Manager.
This is an exciting and worthwhile opportunity to be at the heart of shaping new supported living services across Northern Ireland. You’ll play a pivotal role in helping people with a learning disability transition into community-based living, ensuring their voices are heard and their needs are met.

About the Role
As the Transition Manager, you will be the key point of contact for setting up new supported living arrangements. You’ll bring together families, health and social care professionals, housing providers, and internal teams to co-design person-centred support plans and ensure smooth transitions.
You’ll:

- Lead the development and implementation of supported living transitions.
- Coordinate across multiple stakeholders to ensure timely and effective delivery.
- Champion the rights and choices of people with a learning disability.
- Work closely with our strategic partners to embed best practice and innovation.

What We’re Looking For

- Experience in social care, housing, or community development.
- Strong project management and stakeholder engagement skills.
- A commitment to inclusion, dignity, and person-centred approaches.
- Ability to work independently and collaboratively across teams.
