An exciting opportunity has arisen to join our Initial Contact Team within the Health and Social Care Partnership, Adult Services. Working across East Renfrewshire you will be working within a multidisciplinary team responding to our residents’ health and social care concerns and queries. This will involve managing a caseload, screening, assessing, and reviewing individual cases in line with our Supporting People Framework, and ensuring individuals get access to all of the appropriate services available to them, their carer’s and families.

You will have a good understanding of the current health and social care landscape; self-directed support; the value of outcomes focussed supports for people to enable them to live in their communities and recognise the value of forming effective relationships with other professionals and agencies. This includes the importance of our third sector and community partnerships, and the key role they play in supporting individuals across East Renfrewshire.

We are a modern and ambitious Health and Social Care Partnership who are focussed on the people of East Renfrewshire achieving their full potential, regardless of any additional barriers.

This post is considered Regulated Work with Children and Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007.
