Slough - Qualified Social Worker – Community Team for People with Learning Disabilities (CTPLD)
Intro:
Are you a passionate and dedicated Social Worker looking to make a difference in the lives of adults with learning disabilities? 3D Recruit is working in partnership with Slough Borough Council to recruit an experienced Social Worker to join their dynamic, multi-disciplinary Community Team for People with Learning Disabilities (CTPLD), based at Observatory House.

Key Responsibilities:
Undertake Care Act (2014) assessments, risk assessments, and support planning to identify and meet eligible needs.
Complete Mental Capacity Act (2005) assessments and make best interest decisions where appropriate.
Manage complex care and support cases, including reviews and safeguarding enquiries.
Contribute to Continuing Healthcare (CHC) processes, including checklist completion and multi-agency discussions.
Prepare and submit Community Deprivation of Liberty (DoL) applications.
Work collaboratively with health and social care professionals to promote person-centred, outcome-focused support.
Essential Requirements:
Qualified Social Worker with SWE registration.
Previous experience working within an Adult Learning Disability team.
BIA qualification preferred.
Familiarity with Liquid Logic (LAS) case management system is desirable.
Strong knowledge of UK social care legislation and practice.
Right to work in the UK.
Working Arrangements:
Full-time – Monday to Friday, 9:00 to 17:00.
Hybrid model: Minimum of two days per week in the office, including participation in the duty rota.
