Summary
Cheshire East council is looking for two enthusiastic and committed individuals to join our adult social care community teams for 12 months as social care assessors.
The successful candidate will provide vital support to families, carers and adults with care and support needs. You will join a busy front door multi-disciplinary team of social workers, social care assessors, OTs and Local Area Coordinators, with a mix of home and office working.
We have two full time posts, one based in the East of the area (Wilmslow, Macclesfield, Knutsford) and one in the South (Crewe, Nantwich, Sandbach, Congleton).
As a social care assessor, you will be working alongside other social care colleagues in a busy front line team. You will develop knowledge of local community resources and work effectively with partnership organisations.
You will have experience of working with vulnerable people and their carers in a health or social care setting.
Within the framework of legislation and agreed Council policies and procedures you will work in a strength based and person centred way to support families and carers under the Care Act.
