Responsibilities
Conduct comprehensive assessments of clients' occupational needs and abilities.
Develop and implement individualized treatment plans aimed at enhancing clients' functional capabilities.
Facilitate access to necessary services, supports, and resources for clients.
Provide ongoing case management and coordination of care for assigned clients.
Collaborate with multidisciplinary teams to ensure holistic support for clients.
Educate clients and their families about occupational therapy processes and available resources.
Monitor clients' progress and adjust treatment plans as necessary, ensuring effective interventions.
Requirements
Master's degree in Occupational Therapy from an accredited program.
Current and valid licensure as an Occupational Therapist.
Proven experience in case management or working with diverse populations.
Strong understanding of rehabilitation principles and occupational therapy practices.
Excellent communication and interpersonal skills, with the ability to build rapport with clients.
Demonstrated ability to work collaboratively in a multidisciplinary team setting.
Strong organizational skills and attention to detail, with the capacity to manage multiple cases effectively.

Hours Per Week: 37.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £22.74
