Summary
The primary role of this post is to lead and provide a comprehensive, high quality clinical pharmacy services to the Emergency Department and Acute Medicine. This includes delivering high quality, cost-effective, safe, efficient and timely pharmacy services for patients and wards within your speciality at University Hospitals Coventry and Warwickshire NHS Trust. You will be responsible for developing and advising on pharmacy services that affect both the admission and discharge phases of a UHCW patients journey. This will mean working with teams across the Trust and outside to ensure our patients medications are managed well. This will include screening all TTOs at ward level, supporting the Emergency Department, Medical Admissions Unit, Acute Frailty service and leading/organising a pharmacy team to meet the demands of the service. You will be required to develop and monitor key performance indicators that are patient outcome orientated A significant part of the role will be working with Clinical Service Leads, the pharmacy governance team, other clinical pharmacists and the Operational team to ensure that a suitably trained and equipped team of pharmacists and technicians are well led to provide ward based, patient centred models of care in the ED and Acute Medical department. Part of your role will be to act as a mentor for the more junior pharmacists. As such, you will be responsible for the development and delivery of training and assessments of the rotational pharmacists, Pre-Registration pharmacists and will have direct line management responsibility for the Specialist Pharmacist for Acute Medicine. You may also need to act as a Pre-Registration Tutor, or a clinical coordinator for Diploma Students, if required. In addition to providing excellent care, you will also develop your team and areas to ensure outstanding patient experience. Main duties and responsibilities include: Provide effective clinical and medicines management service to patients admitted and discharged through the Emergency Care Department of the Trust. Liaise with Pharmacy Managers/Team Leaders, Senior Clinicians and Senior Nurse Managers in these specialist areas and in primary care to co-ordinate and ensure all services from pharmacy provide the best in seamless pharmaceutical care for these patients innovating service change when necessary to achieve this. Pro-actively influence safe and cost-effective drug therapy with other senior clinical staff within your areas. This will include new drug evaluations, financial reporting and development of policies. Identify areas within Emergency Care and Acute Medicine services where improvements in both quality, safety and cost effectiveness of pharmaceutical care and prescribing may be encouraged. Work with senior staff in your areas to review and develop policies and procedures to ensure safe and effective handling of medications. Ensure policies and procedures comply with existing legal requirements, best practice, national and local guidelines and Health Care Commission Requirements. Undertake risk management and develop audits relating to supply, storage and administration of medicines and other pharmaceuticals. Lead, develop and deliver education and training in the pharmaceutical aspects of Emergency Care and Acute Medicine to other healthcare professionals. This will include pharmacy trainees and diploma students. To encourage and cultivate an environment of clinical and practice-based research and audit. To contribute to current and future research and audit projects associated with the use of drugs within your areas.
