The Community Therapy Team work with people with a wide range of disabilities and acute or long term conditions striving to deliver excellent patient centred care. Our work is consistently varied and stimulating covering admission prevention and supporting discharge, and rehabilitation in a range of community settings through medical, social and public health approaches. The Community Therapy Team works within the broader Health and Social Care team alongside colleagues from Community Nursing and Adult Social Care. We are a positive and proactive team and adapt to the changing needs of the patients we work with and the service demands. Working Pattern: Various roles aprt time and full time all including participation upon 7 day service rota.
