Summary

Job Summary (Paramedic – Primary Care Network)

An exciting opportunity has arisen for a Paramedic to join a forward-thinking Primary Care Network (PCN) as part of a multidisciplinary paramedic team. The role is supported and mentored by an Advanced Clinical Practitioner (Senior Paramedic) and the leadership team, providing a structured environment for professional growth and clinical development.

The successful applicant will work in a patient-facing role, supporting GP practices within the network, while being encouraged and supported to develop towards Master’s level or equivalent capability in paramedic practice. Training support will also be provided to complete the clinical pillar competencies of the FCP/AP Roadmap to Practice.

Applicants should hold a Bachelor’s degree (Level 6 qualification) in Paramedic Science or equivalent, and be willing to undertake or be completing Postgraduate Diploma level training (PG Cert Paramedic First Contact Practitioner / MSc Advanced Clinical Practice).

Main Duties of the Role

  • Assess, triage, and manage patients, including same-day triage where required.


  • Provide definitive treatment within scope or make appropriate referrals to clinicians or external healthcare providers.


  • Offer advice on general health, wellbeing, and self-management.


  • Promote self-care and signpost patients to relevant community or voluntary services.


  • Manage chronic disease areas, conduct clinical assessments, and deliver diagnostic and treatment interventions either in practice or at patients’ residences.


  • Interpret and act upon diagnostic findings in line with clinical guidance (e.g. NICE).


  • Support practice staff in prioritising and triaging patients appropriately, including in emergency situations.


  • Provide clinical leadership as part of the paramedic team, contributing to quality improvement and service enhancement.


  • Work collaboratively with community and healthcare partners to improve access, efficiency, and patient outcomes.


  • Maintain accurate, systematic records of all patient interactions, interventions, and outcomes.


  • Provide health education, disease prevention advice, and lifestyle support to patients and carers.

    Job Description

    Role Overview

    The post holder will:

    • Act as a registered Paramedic working within professional boundaries and guided by senior clinicians.


    • Take clinical responsibility for patient assessment, diagnosis, and management.


    • Contribute to service improvement and delivery within general practice and the wider community.


    • Support quality and outcomes framework (QOF) initiatives to enhance patient services.


    • Ensure effective integration between primary care, community, and secondary care services.


      Responsibilities
      • Undertake holistic patient assessments and develop treatment plans.


      • Provide appropriate clinical interventions and referral pathways.


      • Recognise and manage risk, ensuring patient safety and effective escalation when needed.


      • Support practice teams in delivering safe, efficient, and evidence-based care.


      • Participate in audits, governance meetings, and continuing professional development.


      • Engage in clinical leadership activities to improve service quality and outcomes.


        Person Specification

        Qualifications (Essential)
        • Professional registration with HCPC (no current investigations).


        • Bachelor’s Degree (Level 6) in Paramedic Science or equivalent.


        • Completion of a two-year Consolidation of Learning period post-qualification.


        • Minimum of three years’ experience as a Band 6 (or equivalent) paramedic.


          Qualifications (Desirable)
          • Postgraduate Diploma or equivalent specialist training.


          • Independent Prescriber qualification (or working towards).


          • MSc or equivalent Advanced Clinical Practice education.


            Experience (Essential)
            • Excellent written and verbal communication skills.


            • Ability to convey complex and sensitive information clearly to patients and professionals.


            • Competence in medicines management and optimisation for long-term conditions.


            • Strong IT skills and ability to analyse complex clinical information.


            • Effective prioritisation and problem-solving under pressure.


            • Demonstrated accountability and professional responsibility in clinical practice.


            • Ability to work autonomously and as part of a multidisciplinary team.


              Experience (Desirable)
              • Report writing and data presentation.


              • Experience in producing service improvement documentation.


              • Valid driving licence and ability to travel between practice sites.


              • Up-to-date Information Governance training.


                Other Requirements

                • The role is based primarily within one GP practice but may involve travel across other PCN sites.
