Summary
Within this role you will provide high quality project, service, initiative and administrative support including information and analysis, that supports the Complex Case Team, with Acquired Brain Injury and Interim Funding complex care. The post holder will be responsible for assessing and reviewing individuals as part of an MDRT who are within funding streams prior to Continuing Health care Process Commencing, hence patients not currently stable or at base line ready for a checklist. Working within the complex case team and/or within the remit of the National Framework (NF) for CHC and FNC, the post holder will provide a high quality service that supports the delivery of an effective and competent service that consistently delivers an individual-focused intervention with patients,. The post holder will support the team leads to undertake reporting and analysis of information to support delivery, ensuring patient are cared for and safe in the least restrictive environment, to achieve their rehabilitation, or interim care goals, to attain their optimum baseline. The post holder is a key member of the team whose overarching goal is to enable, promote and support the effective use of data, information, knowledge and technology to improve, inform and support a portfolio of projects, services, and initiatives.
