Summary
To provide high quality PT - OT management to patients at Imperial College Healthcare NHS Trust. To participate in, providing a high standard of care to medically optimised (or soon approaching) patients within the Emergency Department, Acute Medicine, Specialist Medicine or Elderly Medicine wards as priority caseload dictates. To perform advanced physiotherapy or occupational therapy assessment of patients with diverse presentations and complex physical and psychological and cognitive conditions, to develop and deliver individualised treatment programmes and discharge plans where required. Deliver effective communication with patients, carers, family, colleagues, and external agencies. To be responsible for own caseload and be responsible within a defined area of the service and to undertake all aspects of clinical duties as an autonomous practitioner Maintain accurate patient records and activity data in line with Trust standards. Uphold professional ethics and respect diversity in all aspects of care. Manage time and resources effectively, ensuring reliable attendance and adherence to policies.
