Summary

The post holder will support local community multi-disciplinary teams to successfully discharge children and young people from hospital and live in the place they call home and to work proactively to help them to stay out of hospital. The post holder will demonstrate expert and effective practice in complex situations, working with and supporting colleagues to mitigate high levels of risk. Main duties of the job Enabling preventative and integrated approaches which support children and young people with LD and/or Autism and behaviour that challenges to remain in their local communities by ensuring CETRs are carried out on time and to a high standard and ensuring that admissions to residential or CAMHS inpatient units will be prevented through quality risk management processes. Establish and maintain effective links and good communication with all stakeholders including Local Authorities, NHS England, specialist services, Care Provider organisations and specialist mental health and learning disability service providers (NHS and independent sector). Support processes and mechanisms that will ensure that patients in specialist mental health/learning disability services receive high quality and clinically appropriate treatment in accordance with identified individual care pathways and in the least restrictive setting. Support the commissioning of individual packages of care in the community and monitor assurance outcomes whilst working to ensure individuals are safe and packages of care meet their needs. Provide clinical case management for existing and new transforming care patients, including but not limited to unscheduled participation in CTRs to facilitate crisis intervention. Work collaboratively with stakeholders to agree appropriate care and treatment needs and facilitate timely discharge.
