Summary
Conduct comprehensive frailty assessments and create personalised care plans. Undertake regular care home rounds and domiciliary visits. Prescribe and review medications in line with clinical guidelines and patient needs. Lead on anticipatory care planning and end of life discussions. Work collaboratively to reduce avoidable hospital admissions. Support MDT meetings and contribute to care home alignment projects. Deliver holistic care with a focus on independence, dignity, and quality of life. Ability to work autonomously and with the MDT to manage acute and chronic conditions.
