Summary


The Cancer Support Worker team at UHBW are looking for an enthusiastic and motivated person to join our compassionate & well established team of cancer support workers, who provide personalised support and information to people living with and affected by cancer.

Our cancer support workers work closely with all members of the cancer clinical teams and cancer services across the Trust, providing co-ordination & delivery of support and signposting to additional services.

This provides a fantastic opportunity for you to contribute to improving the overall experience of cancer care at UHBW and work within a friendly and supportive team with an interdisciplinary approach to delivering personalised care.
• Provide telephone advice and refer on or signpost to other sources of support
• Demonstrate the ability torecogniseand respond appropriately when faced with a sudden deterioration or an emergency situation, alerting the clinical team or enabling rapid response as appropriate
• Guide people through the use of self-management resources
• Complete and contribute to the holistic needs assessment and the development of an individual care plan
• Make outbound telephone calls to patients to assess ongoing needs to enable a proactive prevention approach
• Support people to access appropriate information and support, by sign-posting to a range of support services and take an approach which helps people to self-manage where appropriate
• Keep up to date with relevant information and contacts with local services
• Encourage and support active and healthy lifestyle choices
• With guidance from the clinical team, coach patients andcarersto understand what future signs, symptoms or situations to be aware of that would indicate concern and how to make contact when they feel that their condition or needs have changed, including what to do out of hours
