An opportunity has arisen for enthusiastic and committed individuals to join the Community Resource team as Dementia Social Care Practitioner. Successful applicants will form part of a ‘team around the individual’ which will help those living with dementia, their families or carers to access the support/information/guidance that they require.

Practitioners will join a multi-disciplinary team in a particular area of the county. They will provide person centred, outcome focussed support to citizens who are living with dementia and be part of a dynamic and responsive team.
