Summary
Please refer to the Job Description for full details. The post holder will ensure that all care plans are in place which are up to date and support the clients current care needs. They will also ensure there is a regular review process in place for the care plans and where these have not been updated or are omitted from the clients file, they will ensure they are in place promptly. Further to this, the individual will ensure that where a need for on-going referral to another specialist is required (e.g. DOLs, Safeguarding, SALT, OT, CHIT, Community LD specialist Team), that this is completelypromptly, with the outcome of any referrals being included in the CHC assessment asdeemed appropriate. The post holder is to act as an advocate for the vulnerable, challenging poor practice and discriminatory attitudes and behaviour relating to their care. This will include bothCHC Senior Nurse Assessor Band 7 challenging the situation at the time, and also reporting it to an appropriate person or authority as necessary. This is not exclusive to the client being assessed. Case load management Duty Managing small team of CHC assessors timely work planning, KPI, performancemanagement Escalate issues with performance to their senior Support juniors when undertaking challenging assessments, provide clinical supervision and leadership Focused on quality and CG and ensure it is embedded in the day to day practice of the team
