Summary

Advise and contribute to the delivery of high-quality nursing care for patients with physical health issues within the forensic service. Lead on the development and delivery of high-quality nursing care plans for patients with our services and advise on physical healthcare including diet, weight gain, exercise, lifestyle, sexual health, smoking and medication side effect monitoring. Develop, co-ordinate and contribute to the delivery of physical health and well-being training programmes and educational materials for staff in collaboration with senior members of the services. To be conversant with, Adult Safeguarding, NSF, Mental Health Act, Mental Capacity Actand key national Mental Health policies. Provide a role model in the use of evidenced based practice, through working alongside colleagues sharing knowledge,attitudeand clinicalexpertise, and bymonitoringthe effectiveness of care within the service. Partake and lead on audits and research involving the physical wellbeing of patients throughout the service. Working with senior personnel on ensuring that all services have systems in place for monitoring and recording physical health and well-being of the patients they work with. Provide clear clinical leadership that inspires and motivates others through coaching, modelling,trainingand clinical teaching. Undertake clinical work,trainingand personal development tomaintainand develop clinical nursing practicein accordance withthe NMC Code and Mental Health Legislation, which incorporates and respects differences in gender, sexuality, ethnicity, religion,cultureand disability. Contribute to complex case discussions and the investigation of serious Incidents and complaints. Contribute with data and information to draft regular reports providing an overview of the physical health care standards within the service. Toparticipatein effective nursing assessment of care needs, providing a high standard of care to patients in the unit, in keeping with the philosophy and operational policies of the unit (trauma informed care) in addition to Trust policies and procedures. Monitor the effectiveness of care delivery and ensure clinical records are of a high standard. Continuously evaluate care needs, assessing status of patients e.g. physical health status and update care plans regularly and accurately and using FACS criteria. Ensure that all patients have their after-care planned and documented effectively, in partnership with them and their external care team e.g. GP, CMHT, or Addictions services. Work closely and openly with relatives and carers providing help, guidance,supportand information whenever possible. To act as an advocate for patients, within good practice guidelines, ensuring that they understand their choices. Facilitate and co-facilitate a range of healthcare clinics or therapeutic groups, designed to support and educate patients, including pre-session planning and post-session evaluation. Supervising and administering medication as necessary,observingeffects and side effects, regularly updating knowledge on medication; also, working along-side pharmacy colleagues ensure the Medication Storage and Administration Policy is strictly adhered to. Toparticipatein the monitoring and maintenance of standards,in accordance withTrust and local policies and procedures, Clinical Governance and NMC professional documents. Participate in the maintenance of general and specific patient observations, accurately recording all relevant information, and in risk assessments asrequired. Develop, implement & evaluate programmes of care using a problem-solving approach that ensures the needs of each individual patients are met throughout their admission, including detailed arrangements for discharge. Toparticipatein supervision and appraisal for self and staff. Attend meetings asrequested.
