Summary

A Vacancy at Rotherham Doncaster and South Humber NHS Foundation Trust.


To support young people with learning disabilities as they transition from children to adult services. Predominantly working with people age 17 ½ and above

The role will involve working closely with education providers, health professionals, families, and carers to promote smooth, person-centred transitions.

The post-holder will provide education and support around health needs, capacity and consent, and annual health checks, and act as a key link between services.

If you don't already have a Level 3 qualification or equivalent experience in a relevant subject, RDaSH are offering this post alongside the opportunity for you to complete a Level 3 apprenticeship qualification [relevant to your role].

As an apprentice within RDaSH you will be given the opportunity to gain experience, learn work-related skills and obtain all the tools needed to thrive.

An apprenticeship includes regular training with a college or other training organisation. Around 20% of your working hours will be spent training or studying. This leads to a nationally recognised qualification.

Whether you already work for us or are joining the Trust and NHS for the first time, apprenticeships can help you to develop the skills, knowledge and know-how in order to embark on a successful and fulfilling career within the health service.



· Under direction of senior community nurse, hold and manage a defined caseload of young people transitioning from child to adult services

· Offer tailored support to each individual, promoting choice, independence, and inclusion.

· Liaise with multi-disciplinary teams, families, carers, and service providers

· Attend transition planning meetings and reviews.

· Deliver and support health and wellbeing workshops in community settings and specialist schools/colleges. Promote awareness and understanding of health services, and healthy lifestyles.

· Build strong working relationships with local specialist schools and colleges.

· Provide in-reach support, including drop-in sessions, group work, and individual preparation for adult services.

· Act as a visible and approachable link for students, staff, and families.

· Support individuals to become desensitised to health procedures such as having blood tests or measuring vital signs.
