Summary
Lead Multi-Disciplinary Team (MDT) meetings. Completing either the NHS Continuing Health Care (CHC) Decision Support Tool or Adult Needs Assessment with clear eligibility recommendations. Completing CHC reviews. Manage relevant follow up calls and request further documentation and evidence as required from other professionals and care providers. Conduct the assessments, reviews, and any care and support plan, working to both Liaison Care and the ICBs Key Performance Indicators (KPIs).
