Summary


An exciting opportunity has a risen for a Band 4 Coordinator to join our team

The post holder should be a creative and clear thinker, with administrative experience and with excellent skills with technology to help manage our Community therapy team referrals.

Your role would involve :

· Processing community therapy team referrals

· Phone call with patients, staff, health care professionals

· Working collaboratively as part of a multidisciplinary team

· Through close liaison with the Team lead, to manage the team waiting lists, allocating visits contacting patients, booking appointments

To ensure that an accurate and up-to-date patient record is maintained by updating our clinical system as necessary. Training will be provided.
