Summary
An excellent opportunity has arisen to become atriage coordinatorin a friendly and supportive team within the BHFT. The successful candidate (s) will be working alongside an established multi-disciplinary team consisting of senior specialist community physiotherapists, Occupational Therapists,therapy assistants, and team administrators.As part of this role the post holder will be an integral member of the Clinical support and triage team.We are an integrated team including Occupational Therapists, Physiotherapists, Therapy assistants, and admin support. The service supports multiple pathways including CommunityReablement, Falls and Community Physiotherapy. This is an exciting time to join BHFT, as we develop services to work in an integrated way within the community in West Berkshire and across Berkshire West. The successful candidate willbe requiredto provide/ willing to develop skills in providing high quality, responsive services working as an integral member of the team. You will also work closely with social services and voluntaryorganizationfor adults with a wide range of physical impairments and mental health conditions.
• To be an integral member of the triage team – toassistwith the management of referrals, and support the integrated triage process
• To attend and lead the Sitrep flow calls with the Hospital Discharge Team
• To take a lead in theallocationsto support the team.
• To be part of discussionsregardingpatients progress while on the service and review with the multi-disciplinary team asappropriate.
• To have a patient centred approach.
• To liaise with Registered Nurses, Registered Occupational Therapists, Registered Physiotherapists and Care Managers/Social Workers as required and to keepaccurateand concise reports.
• To contribute andparticipatein professional development meetings
• To be the first point of contact for patients and health and social care partners wishing to discuss referrals and to escalate accordingly as and whenappropriate .
• To coordinate adequate therapyassistantscoverage where needed.
• To support the administration team as needed

The “must haves” for you to be considered for this role:
1. Experience of working in health/social care at Band 3 level or equivalent.
