Responsibilities
Conduct comprehensive assessments of clients' needs and strengths
Develop, implement, and evaluate individualized care plans
Provide direct counseling and support services to clients
Facilitate access to community resources and services
Advocate for clients within various systems, including healthcare and social services
Document client interactions and maintain accurate case notes
Collaborate with multidisciplinary teams and external agencies to coordinate care
Participate in training and development opportunities to enhance professional skills
Requirements
Master's degree in Social Work (MSW) from an accredited program
Current and valid Social Work licensure
Minimum of 2 years of experience in a social work role
Strong communication and interpersonal skills
Ability to work effectively with diverse populations
Knowledge of relevant social policies and community resources
Strong organizational skills and the ability to manage a caseload
Hours Per Week: 36.00
Start Time: 08:30
End Time: 17:00
Pay Per Hour: £18.85
