Summary
Key Responsibilities Patient Care Work in partnership with GPs to provide high-quality, responsive healthcare services to patients in their homes, including assessment, diagnosis, treatment, and follow-up care. Collaborate effectively with GPs, nurses, and other healthcare and social care professionals to ensure holistic and coordinated patient care. Assess patients, obtain detailed medical histories, and conduct comprehensive physical examinations during home visits and practice consultations. Analyse, diagnose, and explain medical problems clearly and compassionately to patients and their families. Recommend, arrange, and interpret appropriate diagnostic tests and investigations in line with clinical governance and agreed protocols. Perform specialised diagnostic and treatment procedures within professional competency and supervision requirements. Formulate evidence-based treatment and management plans, consulting or referring to a supervising GP as appropriate. Educate patients and their families about health conditions, treatment plans, and preventative healthcare to promote self-management and long-term wellbeing. Ensure all medical equipment is maintained, clean, and in good working order. Accurately document all aspects of patient care, ensuring legal, clinical, and administrative requirements are met.
