Summary
Clinical Responsibilities Home Visits Attend scheduled and urgent home visits for housebound or vulnerable patients. Conduct comprehensive assessments in the patient's home environment. Manage acute illness presentations and chronic disease exacerbations in the community. Identify safeguarding or social care concerns and liaise with appropriate services. Document visit outcomes clearly in the medical record. Escalate concerns to clinical triage team or patient's own GP when necessary. Professional Duties Maintain professional registration and adhere to regulatory standards. Participate in continuing professional development (CPD) and clinical supervision. Follow local clinical pathways, protocols, and prescribing guidelines. Contribute to service audits, quality improvement, and practice development. Support induction or training of junior paramedics or students (if applicable). Work collaboratively within a multidisciplinary primary care team.
