Summary

Integrate and work within our member practices to enhance and support day to day access and capacity. Provide additional capacity support for the PCN care home team. Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways or make necessary referrals to other members of the Primary Care team. Manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action. Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice. Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person. Complete the relevant training in order to provide multi-professional, clinical practice and CPD supervision to other roles within Primary Care, for example, first contact practitioners and personalised care roles. Undertake clinical audit and quality improvement work across the PCN. Optional for the right candidate: Provide programme leadership for our Cancer and/or Cardiovascular Disease Programmes Provide leadership for practice nursing teams (i.e. the importance of SMI health checks, elderly health checks, LD health checks) Provide support with the PCNs research programme Is responsible for the day-to-day planning of personal workloads Follows organisational policies, protocols, procedures, and standards. Reports any incidents, accidents or complaints in a timely manner, taking appropriate action where necessary Develops themselves and the role through participation in clinical supervision, training and service redesign activities Contributes to a patient safety culture through reporting and investigation of incidents and undertaking proactive measures to improve patient safety Maintains accurate clinical records of all patient consultations and related work Supports in the delivery of enhanced services and other service requirements on behalf of the PCN Participates in the management of patient complaints when requested to do so and participates in the identification of any necessary learning brought about through clinical incidents and near-miss events Undertakes all mandatory training and induction programmes Attends a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed Performs other general tasks as assigned.
