Summary
Our nurses are committed to teamwork, have good communication and decision making skills and consistently work collaboratively with other professionals within HCT and beyond. We aim to provide a high standard of patient care using an evidence-based framework. Successful applicants will be responsible for assessment, planning, implementation, and evaluation ofindividualised nursing care to patients living in the community. They will need to support the Nursing Sisters of the team to manage the caseload and support junior staff. A commitment to health promotion teaching and training is essential, to optimise our patients ability to self-manage their health needs and engage them in maintaining their wellbeing. Word versions of documents are available if needed to allow for any adaptations to be made i.e. font, colour, background, convert to Read Aloud.
