Summary
To case manage an agreed number of Transfer of Care patients, completing initial assessments, care plans and onward referrals referring to senior clinicians for support when required.
• Plan and implement daily work within the team and wider geographical area in the knowledge that this can be unpredictable and may change to meet the demands of the service and the changing priorities of clinical/health needs
• Works within the multidisciplinary team in a primary care setting including clients’ homes, residential homes and clinic,providing support for a variety of therapeutic and clinical interventions based on the team skill mix.
• Supports Integrated working with system partners and services: inclusive of Falls, UCR and Virtual Wards.
• To liaise with multi-agencies to co-ordinate discharges in a timely and efficient manner
• To work autonomously and undertake lone working in the community using your own vehicle.
• To support in-reach processes into Bedfordshire Hospitals.
• To work a variety of shift patterns which include weekends and bank holidays on a rota basis.
• To participate in team planning, by organising new referrals as to who needs to be assessed within an agreed service time frame, and on-going work allocations, to enable the team to work effectively and efficiently whilst supporting the demand within the service.
• To support other members of the multidisciplinary team in planning and managing their own caseload particularly in the event of absence.
• To be responsible for prioritising and managing own workload, expecting some conflicting demands on your time.
• To support the In-Reach model into the Bedfordshire Hospitals, particularly MADE events and in times of service demand.
• To chair multi-disciplinary team caseload meetings and ensure information on individual patients is fed back to the team for planning purposes.
• To have an enhanced level of knowledge of system partners and services.
• To act as named person for the client to contact within the department and to feedback relevant issues to senior staff.
• To provide supervision and appraisals of staff within the team.
• To complete the duty rota for the Transfer of Care element of the locality team.

• Coordinate for the Transfer of Care Service, supporting hospital discharges, Urgent Care Response and Virtual Wards but ensuring clear communication and coordination of the service.
• Support in times of high pressures, reprioritising workloads of self and others to ensure continuous and smooth processes.
• Maintain a strong and professional relationships with systems partners, working in a collaborative, support and professional manner.
• Maintains own caseload of clients for rehabilitation and self management of conditions, providing education and health promotion to the population of Bedfordshire.
• To support management in times of service pressures.
• To have own vehicle to travel the County of Bedfordshire.
