Summary

To work within clinical boundaries, demonstrating safe clinical decision-making, delivering high quality clinical care. To provide care and structured education to patients with diabetes. Become the Diabetes Champion and attend meetings and updates as required. Participate in reviewing Diabetes protocols when required. To provide essential treatment duties such as BP checks, Long Term Condition Reviews, NHS Healthchecks, prescribed injections (e.g. B12, Depo-Provera and Denosumab) BMI and lifestyle advice Provide holistic assessment of wounds and implement wound care including complex wound management in line with current evidence-based guidelines Offer advice about childhood and adult immunisations and ensure vaccines/injections are administered under patient group directives (PGDs) or individual prescriptions (PSDs) Participate in national screening work such as cervical cytology and deliver the national immunisation and vaccination schedule Actively participate in seasonal and special projects as requested e.g. flu campaign Act as chaperone when required Vaccine / cold chain storage, monitoring and recording Undertake health promotion to help educate patients To maintain accurate patient records and enter onto the computer using agreed Read Codes To collect data and participate in regular Practice audit and research projects To contribute to the preparation of any Practice development plans To maintain own annual registration with the NMC and adhere to Code of Professional Conduct To maintain own continuous professional development with the support of the practice as required Attend and participate in staff and Practice meetings and clinical training sessions.
