Summary
To use clinical reasoning and negotiation skills to develop care plans which address the clients goals, clinical need and to communicate these goals effectively. To provide advice and education to client, carers and relatives based on clinical evidence and identified need. To demonstrate clinical effectiveness through evidence-based practice and the use of outcome measures. To work in a co-ordinated, multidisciplinary manner, communicating effectively with other team members and the wider clinical network involved in client care. To triage referrals into the service to ensure the waiting time for assessment is met and allocation to appropriate skilled staff. To be accountable for own professional action and recognise own professional boundaries, seeking advice as appropriate through mentoring, peer support, clinical and managerial supervision. To maintain up to date and accurate case notes in line with RCSLT professional standards and local trust policies.
