Summary
We have an exciting opportunity for someone to join our Hull Home Care team as our Field Care Supervisor!
About the role:
As a Field Care Supervisor, your role will be diverse to each client. You will be responsible for establishing a professional client relationship, undertaking new client assessments and writing person centred care plans. You will be the first face of the business and will need to build and maintain relationships with clients and their loved ones/representatives, as well as colleagues, ensuring staff have effective supervisions, personable support, and their training remains up to date.
Our Field Care Supervisors must have exceptional time management skills, with the ability to prioritise and organise their own workload. A high level of attention to detail is needed, along with a passion for high quality care standards also being critical for this position.

Hours of work:
This is both and office and field-based role and hours may vary to match company needs.
1x part time position 20 hours a week
Shift patterns: Monday - Friday, 9am-5pm
Participating in the service’s on-call rota is a requirement for the role (1 week in every 4)
Job Description:
Writing person centred care plans
Carrying out new client assessments and risk assessments
Undertaking Mental Capacity Assessments and completing Best Interest Decision paperwork
Leading on client reviews and undertaking any resulting actions
Medication reviews and audits
Monitoring the quality of care delivered through spot checks in the field
Delivering care calls when required
On-call duties
Person Profile:
Good knowledge and understanding of working with Local Authority Contracts and CQC legislation
Proficient IT user
Excellent Communication skills
Ability to work collaboratively as part of a team
High attention to detail
Passion for high quality and care standards
Computer literacy to include Microsoft Word and our own roster system (training provided)
Experience and Requirements:
Have a right to work within the UK
A driving licence and access to a vehicle
Minimum NVQ/QCF Level 3 or equivalent in Health and Social Care
Minimum 1-year Domiciliary Care experience
Previous experience of writing care plans -desirable
Experience of conducting risk assessments desirable
Experience with Access Care Planning Software would be beneficial, but not essential
You will need to demonstrate real compassion, be an excellent communicator and problem solver, and be committed to delivering high-quality, respectful care
