Summary
Complete comprehensive mental health assessment of the needs of service user. Responsibility for managing an allocated workload, providing agreed intensive levels of support and interventions and establishing priorities within it. To ensure that the care plan is fully implemented with the service user in accordance with service specifications including liaising with and providing information to other agencies involved in the care of the service user. Monitor the quality of care delivered and service user and carer satisfaction with the service. Assist in reviews of service user in the community on a systematic basis to ensure the continued need and appropriateness of services provided by the Trust and maintain review records. Adhere to, and where appropriate, ensure the implementation of relevant legislation, Department and Trust policies and procedures Maintain accurate and up-to-date records as required by the Trust. Participate in a formal programme of supervision.
