Summary


An exciting opportunity has arisen for a Community Staff Nurse to join the Haywards Heath Central and Villages PCN Community Team.

The PCN is a group of practices collaborating with community and other health and care services. PCNs enable more proactive, personalised, coordinated, and integrated health and social care. Care is organised around individuals' needs through collaborative working to deliver excellent patient care at the heart of the community.

Haywards Heath PCN Team is looking for registered nurses to join. Whether newly registered or seeking a new challenge, Community Nursing offers a chance to specialise in a highly diverse and rewarding field.

Community nursing provides many specialties in one role.
Successful applicants will receive an individual induction plan to ease into this environment.

To deliver high quality care in the community in patients own homes, working with patients towards rehabilitation, self-management and independence throughout the continuum of care. To participate in a team of colleagues supporting evidence based care, facilitating staff learning and development.

The main aspects of the role are diabetic management, wound management including leg ulcer diagnosis and treatment, palliative care working very closely with the Hospice and GP’s, bladder and bowel care including urethral and supra pubic catheters and other continence needs.
