Summary
Main Duties of the Role As part of our multidisciplinary team, you will contribute to a wide range of medicines optimisation and patient safety initiatives. Core duties will include: Supporting structured medication reviews and medicines reconciliation Promoting safe and effective repeat prescribing processes Liaising with patients to improve their understanding and use of medicines Working with community pharmacies and hospitals to improve continuity of care Assisting with prescribing audits, data analysis, and quality improvement projects Supporting the practice in reducing medicines waste and improving prescribing outcomes
