Summary
The following are the core responsibilities of the Medical Summariser. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels Key Responsibilities: Ensure accurate SNOMED coding, covering QOF, IIF, and Enhanced Services, to facilitate patient recalls and maintain comprehensive medical records. Summarise the medical history of all new patients, both from paper files and electronic sources. Prepare and summarise patient records for anyone requesting Online Access before access is provided. Open, organise, scan, and code all incoming correspondence, including referral letters and confidential paperwork. Manage clinical correspondence by directing it to the correct departments. Process Out-of-Hours documentation and prepare it for review by clinicians. Chase up incomplete medical records and contact external practices when further details are required. Keep existing patient files accurate and up to date, making changes to diary entries where necessary. Monitor the GP2GP inbox and ensure records are prepared for scanning and summarisation. Undertake assigned duties and provide support with any other reasonable tasks as needed. Repeat medication requests handling and process them
