Summary
As a Diabetes Specialist Nurse within the Intermediate care service the post holder will be responsible for delivery of safe and effective care and support of patients living with Diabetes. Educational provision in the care of diabetes, both internally and externally to the Trust is equally an essential elementThere is an expectation to support the County wide model of care whilst holding a case load within a specific locality linking with community neighbourhoods.
