Summary

Clinical Care & Patient Support Assess and treat patients using expert knowledge of medicines. Prescribe (or train to prescribe) and support the general practice team. Manage care for patients with long-term conditions, complex polypharmacy, frailty, asthma, COPD, learning disabilities, or autism. Conduct structured and new-patient medication reviews. Provide proactive care for vulnerable or high-risk patients, helping prevent admissions/re-admissions. Support patient/carer understanding, confidence, and adherence to medicines. Signpost patients to the right healthcare professional. Answer medication queries from patients and staff. Medicines Optimisation & Prescribing Lead on person-centred medicines optimisation, including antimicrobial stewardship. Encourage safe, effective, and cost-efficient prescribing across the PCN. Implement and embed robust repeat prescribing and medication monitoring systems. Handle prescription queries and requests directly. Supervise safe storage, rotation, and disposal of vaccines and drugs in line with infection control standards. Promote self-care and reduce medicine waste. Leadership, Collaboration & Integration Provide leadership on prescribing systems, quality improvement, and service delivery. Work as part of a multi-disciplinary team and integrate with wider healthcare services (community, hospital, mental health pharmacists). Develop strong relationships across PCNs and health/social care networks. Support and mentor junior pharmacists through structured supervision. Contribute to service development and enhanced services delivery. Quality, Governance & Education Improve prescribing quality through audits, NICE/CQC compliance, and clinical guidance. Contribute to clinical governance, patient safety, and complaint management. Learn from incidents/near misses and embed improvements. Support education of staff on prescribing and medicines use. Participate in research, shared care protocols, and public health campaigns (e.g. flu, COVID-19). Professional Development & Admin Undertake mandatory training and appraisals. Engage in regular clinical supervision (monthly for clinical pharmacists, quarterly for senior/advanced). Maintain an effective, clean, and safe working environment. Contribute to performance reviews and continuous improvement.
