Summary
DUTIES AND RESPONSIBILITIES 1) Provide evidence based individualized patient care according to patients changing healthcare needs, through electronic holistic needs assessment. 2) Participate in the clinical management of patients and formulate clinical management plans as per medical instruction and patient agreement. 3) Act as a `Keyworker` for a clinical case load and facilitate the co-ordination of patient care. 4) Support the haematology team to assess patient`s information needs and provide relevant information to meet those needs, ensuring the focus is on the promotion of health as well as illness.
