All 2 year fixed Term Contracts - 2 posts 30 hours, 1 post 37 hours per week

We are pleased to be able to offer three new rewarding opportunities to join our Lily team for the newly launched Proactive Intervention Service. This innovative initiative aims to prevent, reduce, and delay the need for statutory and NHS services by supporting adults in West Norfolk to age well and live independently. As a Lily Proactive Intervention Support Officer, you will play a vital role in helping residents access a wide range of services and activities that promote wellbeing and reduce the risk of falls.

In this role, you will conduct holistic telephone triages with pre-identified clients and, where appropriate, visit them in their homes or community settings to offer tailored advice and support. You will help clients identify their needs and connect them with internal and external services, while also maintaining and updating the Lily Directory to ensure it remains a trusted and comprehensive resource for residents of all ages. Your work will involve close collaboration with colleagues across the council and external partners in health and social care.

We are looking for someone with a caring and sensitive attitude, excellent communication skills, and a commitment to providing high-quality customer service. You should have 5 GCSEs including English and Maths (grade C or above), experience working with older or vulnerable people, and confidence using IT systems and databases. A full clean UK driving licence and access to a vehicle with business insurance is essential, as the role involves travel across the borough.
