Summary
Undertake the named nurse role for patients undergoing renal replacement therapy at home Responsible for the teaching/ implementation and evaluation of their treatment for a group of patients out in the community Actively contribute to setting and maintaining high standards ofquality nursing care. Work collaboratively and co-operatively with others to meet theneeds of patients and their families. Act as a clinical and professional role model, Adhere to Trust policies, procedures and clinical guidelines and ensure compliance of others.
